- Do not assume a complaint pauses another legal route.
- Use documents and dates rather than unsupported conclusions.
Identify the responsible service
An operational prison decision, NHS healthcare decision and probation decision may involve different records and escalation routes. Name the responsible body for each complaint point.
Use the internal complaint route
Keep copies or photographs of complaint forms and responses. Record submission dates and any reason an urgent or confidential route was required.
Request the underlying record
Identify logs, healthcare contacts, property records, decision forms or policies likely to show what occurred. A missing record should be described specifically.
Frame fault and consequence
For each issue, state what happened, what should have happened, the proof and the consequence. Keep urgent clinical risk separate and seek immediate healthcare or legal help where required.
Prepare a concise Ombudsman referral
Explain exhaustion of the internal route, attach final responses, number the complaint points and specify a realistic remedy. Do not require the investigator to reconstruct the chronology from loose papers.
A reliable challenge structure
- The decision and date.
- The applicable rule, policy or standard.
- The finding disputed.
- The evidence supporting your correction.
- The procedural failure, if any.
- The practical impact.
- The precise remedy requested.
