How we review a decision
1. Triage the route and deadline
We begin with the operative decision, the date it was received and the outcome sought. We identify whether the issue is a complaint, internal review, statutory appeal, regulator referral or potential legal matter. Where an authorised lawyer is required, that is made clear early.
2. Control the documents
Files are organised by date, source and issue. Duplicate material is removed from the working record. Missing attachments, unexplained gaps and references to records not supplied are logged.
3. Rebuild the chronology
A reliable chronology records what happened, who knew what, when decisions changed and whether promised actions occurred. It distinguishes evidence from allegation and records uncertainty rather than filling gaps with assumptions.
4. Test policy, process and evidence
We map each material conclusion to the rule or criterion said to support it. We then test whether relevant evidence was considered, irrelevant material influenced the outcome, reasons are intelligible and the procedure gave a fair opportunity to participate.
5. Produce focused work
The output depends on the matter: an issue schedule, evidence matrix, chronology, formal complaint, review request, response to findings, Ombudsman pack or briefing for an authorised professional. The requested remedy is tied to the fault and impact identified.
